What documents need to be provided for the purpose of social payments?

The appointment of social payments is carried out on the basis of application (including electronic), and the documents proving the identity (for identification);

and also:

1) in case of loss of the breadwinner;

certificate of death of the supporter or of a court decision declaring a person missing or dead;

documents confirming family relationship with the deceased (recognized by the court as missing or declared dead), certificates of marriage (marriage), birth of children of the deceased breadwinner, on adoption (adoption);

certificates from the organizations of General secondary, technical and vocational, secondary and higher education that family members are students or students studying in full-time education (updated annually) in the prescribed form;

of the document confirming adoption (adoption) (if necessary);

2) in case of loss of income in connection with pregnancy and childbirth, adoption (adoption) of a newborn child (children):

sheet (sheets) of temporary disability, issued in connection with pregnancy and childbirth, adoption (adoption) of a newborn child (children);

certificates (certificates) from the work place about income for the last twelve calendar months preceding the month in which occurred the right to social payment in case of loss of income in connection with pregnancy and childbirth, adoption (adoption) of a newborn child (children) in the prescribed form;

for self-employed individuals in the act of reconciliation on taxes and other obligatory payments to the budget, issued by the state revenue authorities, in accordance with Appendix 17 of the Rules of conducting personal accounts, approved by order of the Minister of Finance of the Republic of Kazakhstan from December 29, 2008 № 622, registered in the state Register of regulatory legal acts under No. 5446;

3) in case of loss of income due to care for child reaches one year of age:

certificates (certificates) on the birth of a child (children) or extract from Assembly record about a birth;

extracts from the court decision on adoption (adoption) of the child (Ren), issued by the authority, carrying out functions on guardianship or custody (in cases of adoption (adoption) of the child (children) under the age of one year).

Thus, for the purpose of social benefits submission of documents, birth certificate of the child (children) or extract from Assembly record about a birth of the registrations made in the territory of the Republic of Kazakhstan after August 13, 2007, certificates of marriage, registration produced in the territory of the Republic of Kazakhstan after June 1, 2008 is not required.

In addition to these documents attached to the application information received from the information systems (the identity of the applicant of registration at the permanent place of residence; the number of the Bank account opened in banks and (or) the organizations performing separate types of banking operations, either the control account of cash of the correctional institution; on the state registration as the individual businessman (if applying for the appointment of social payments in case of loss of income in connection with pregnancy and childbirth, adoption (adoption) of a newborn child (children) an individual entrepreneur); the establishment of guardianship (guardianship) over the child; in case of disablement – establishing the degree of loss of work capacity. in case of loss of breadwinner, holding of examinations and the disability (in the case when children, including adopted (adopted) brothers, sisters and grandchildren under eighteen years of age and older recognized as disabled from childhood group I or II); in case of job loss – on the registration as unemployed by the employment center).

In case of discrepancy between (lack of) information in information systems to the application shall be accompanied by the following documents:

a copy of the identity document (identity card, certificate of a stateless person, residence permit of the foreigner);

a document confirming the registration at the permanent place of residence (address reference or reference rural akims for the residents of the city of Baikonur – the certificate of the division for accounting and registration of citizens of housing of the city of Baikonur Russian Federation);

data on Bank accounts opened in banks and (or) the organizations performing separate types of banking operations, either the control account of cash of the correctional institution;

information about the establishment of guardianship (guardianship) over the child.
